    Abstract [en]

    Today, many of the 10 million childhood deaths each year are caused by diseases of poverty--diarrhea and pneumonia, for example, which were previously major causes of childhood death in many European countries. Specific analyses of the historical decline of child mortality may shed light on the potential equity impact of interventions to reduce child mortality. In our study of the impact of improved water and sanitation in Stockholm from 1878 to 1925, we examined the decline in overall and diarrhea mortality among children, both in general and by socioeconomic group. We report a decline in overall mortality and of diarrhea mortality and a leveling out of socioeconomic differences in child mortality due to diarrheal diseases, but not of overall mortality. The contribution of general and targeted policies is discussed.

    Abstract [en]

    As in European countries a century ago, diarrhea is a major cause of child mortality in poor countries today. In Stockholm at the turn of the 19th century, political commitment, infrastructural investments in water and sanitation, and enforcement of sanitary improvements by a strong implementing organization helped eliminate diarrhea as a principal cause of death among children. These interventions also had an equitable impact on social class differences in diarrhea mortality, but not on overall mortality; overall mortality declined, but class differences remained. General infrastructural improvement and health education coupled with targeted interventions to vulnerable children may be successful in improving child health and reducing social differentials in mortality. Specific health care interventions may need to be complemented by infrastructural investments to improve water and sanitation if diarrhea mortality is to be further reduced in poor countries today.

    BACKGROUND: At the end of the 19th century, infant mortality was high in urban and rural areas in Sweden. In Stockholm, the mortality rate was particularly high among foster children. This study addresses the importance for health of targeted public policies and their local implementation in the reduction of excess mortality among foster children in Stockholm at the turn of the 19th century. In response to public concern, a law was passed in 1902 on inspections of foster homes. Stockholm city employed a handful of inspectors who visited foster homes and advised parents on child care and feeding. METHODS: Analysis of historical records from the City of Stockholm was combined with epidemiological analysis of mortality rates and hazard ratios on individual-level data for 112 746 children aged <1 year residing in one part of Stockholm between 1878 and 1925. Hazard ratios of mortality were calculated using Cox' regression analysis. RESULTS: Mortality rates of foster infants exceeded 300/1000 before 1903. Ten years later the mortality rates among foster children had declined and were similar to other children born in and out of wedlock. Historical accounts and epidemiological analysis of individual-level data over a longer time period showed similar results. CONCLUSIONS: Targeted policy measures to foster children may have potentiated the positive health effects of other universal policies, such as improved living conditions, clean water and sanitation for the whole population in the city, contributing to an equalization of mortality rates between different groups.

    Abstract [en]

    This study investigates differentials in the decline of cause-specific infant mortality by marital status of the mother in Stockholm (1878-1925) and factors contributing to the explanation of these differentials using computerized records of individual entries from the Roteman Archives. Included in the analysis were 120,094 children less than 1 year of age who lived in Södermalm during this period. Cause-specific mortality rates were calculated for three time periods. Cox's regression analysis was used to study the relationship between overall and cause-specific risk of infant death and of being born in and out of wedlock in relation to a set of variables. Infant mortality rates and mortality risks were higher among children born out of rather than in wedlock. The most pronounced differentials in cause-specific mortality rates between these groups of children were seen in cases of diarrhea. The socioeconomic status of the household head and number of children in the household were statistically significant with infant mortality, but explain only part of the excess mortality risk of children born out of wedlock. In Stockholm at the turn of the 19th century being born out of wedlock was strongly associated with poor health outcomes, particularly in diarrheal diseases, pneumonia/ bronchitis, and immaturity/congenital causes.

    Lukter och dofter är gåtfulla - mäktiga men undflyende. En doft avklingar snabbt, även om luktämnet finns kvar i luften, men intrycken lever kvar i vårt inre. Luktsinnet hjälper oss att finna mat och dryck och kärlek, men de dofter som leder oss låter sig ogärna fångas i ord.

    Luktsinnet har betraktats som primitivt, djuriskt. Det förekommer sällan i vetenskapliga studier.

    I Näsans kunskap lyfter fem forskare fram luktsinnet från dess undanskymda plats. De kommer från vitt sklida ämnesområden: konst och litteratur, arbetsliv och yrkeskunnande, filosofi och psykologi. Här presenteras också sensoriken, den vetenskap som utforskar just lukt och smak.

    The visible and the non-visible. An inventory of clothes belonging to deceased patients of the Stockholm maternity hospital 1860-1870.

     

    In mid19th century Stockholm pregnant women who lacked a home of their own usually gave birth at state-owned “Allmänna barnbördshuset”, free of cost. As with most maternity hospitals in Europe in the 1860s this public lying-in hospital had a towering mortality rate, mainly caused by outbreaks of puerperal fever.

    When patients died, their belongings were returned to their relatives. If not claimed within three months, the rest was sent off to the state orphanage to be distributed among the wet-nurses and girls of that establishment. An inventory used for this distribution, compiled between 1860 and 1870, provides unique information about the dress of working-class city women in the wake of the industrial society.

    The inventory shows that the publicly visible dress usually consisted of a skirt, with a jacket, combined with shawls and scarves depending on the season. Dresses were rare compared to skirts. The low number of hats indicates that working-class city women normally still stuck to the headscarf. Among a small number of fashion items appears the bonnet, the “beduin” (a knitted hood with frills) and the “Garibaldi-blouse” (a long shirt with wide sleeves, named after the Italian revolutionary.)     

                          Under the outer skirt women wore one or two more skirts or petticoats. In wintertime, the inner petticoat was occasionally a stubb, a quilted or knitted, tight miniskirt worn for warmth. This item appears more frequently in the inventory at the beginning of the period that at the end, indicating a gradual disappearance of the stubb. The initially small number of drawers listed increases however, suggesting that drawers were gradually replacing the stubb. Nevertheless, women did not normally wear underpants even by the end of the period.

    Joan L Severa comments in her book Dressed for the photographer: Ordinary Americans and fashion 1840-1900 the appearance of knickers in women’s dress in the 1860s in the following way: “Pantalettes seem to have been adopted by mature women by the late fifties, perhaps because of the undependability of large hoops [of the crinoline, LÖ] in certain circumstances. … Information on the wearing of this garment is scarce, however, since pantalettes were never a part of fashion discussion as they contributed nothing to the silhouette, though they are included in descriptions of sets of under-clothing” (Severa 1995 p 201). Similar findings are reported by researchers studying British women’s fashion of the same period.

    To conclude, the inventory offers evidence about the existence and the type of women’s publicly visible as well as publicly invisible clothing from 1860 to 1870, including underwear, worn in an urban, working-class context. Regarding the non-visible, a majority of the deceased women did not wear underpants, although a growing number began to do so during the period. At the same time the use of the traditional short and warm under-skirt, “stubb”, dwindled. These facts indicate a change of habits concerning below waistline underwear, taking place in Stockholm at the same time as in the United States and in Great Britain, or perhaps slightly later. At the time when visible trousers, so called Bloomers, were highly controversial and were being worn only by a limited number of avant-garde females outside Sweden, working-class women silently started to adopt the habit of wearing trousers, in the shape of drawers, under their skirts.
